There are two kinds of noise cancelling headphones, one is the active kind and the others are passive. The passive versions achieve the reduction of outside noise by using padding.

Active noise cancellation has more to it. These kinds of headphones have a system built that produces frequencies to cancel the ones belonging to outside noises, essentially cutting them out for the listener. One of the major advantages of noise cancelling headphones is that a person does need to keep their volume high so they can hear only their song and not the outside. With noise cancelling headphones you could have the volume lower and be hearing only what the headphones are producing; hearing it clearly as there are no intruding sounds. This in the long run means you expose your ears to lower sound levels, causing less ear fatigue.

There is a downside with noise cancelling headphones, but this depends on where one is. For instance, if you are walking by the roadside you may not hear a car coming which could be dangerous. This is why one has to be careful. This is also why Apple added a feature in newer AirPods that mixes in some external sounds when you need so you are aware of your surroundings.
